A report into Britain’s largest maternity scandal has found that babies died because of repeated failures to learn lessons. Investigators found that mothers were often blamed for their babies’ deaths. Some 1,862 cases of stillbirths, brain damage and deaths of babies and mothers are being investigated at the Shrewsbury and Telford Hospital Trust.
