Southport attacker Axel Rudakubana murdered three girls at a Taylor Swift-themed dance class after authorities missed a string of opportunities to identify the risk he posed to others.
Prime minister Sir Keir Starmer has admitted the state failed to protect the three young girls in Southport last summer.
The government has confirmed a public inquiry will look into how authorities failed to stop the killer – despite three referrals to the anti terror programme, Prevent.
