NHS trusts across England are being investigated over failures in maternity services, with experts warning that women and babies are not receiving the "safe, compassionate care they deserve".
Issues dating back more than 15 years are being investigated across 14 trusts – and the review’s chair says the findings will lead to nationwide improvements.
