Southport attack blamed on killer’s parents and four other ‘catastrophic’ failures

Axel Rudakubana would not have carried out the Southport attack if his parents had reported what they knew about their son to the authorities, a public inquiry has found.

Chairman Sir Adrian Fulford has said the murders of Alice da Silva Aguiar, nine, Bebe King, six, and Elsie Dot Stancombe, seven, at a Taylor Swift-themed dance workshop on 29 July 2024 "could and should have been prevented".

Sir Adrian found that Rudakubana’s parents "ultimately failed" to report the risk their son posed – and he also identified several major areas of systematic failure within the police, security, health and social care services.
