Nottingham maternity scandal: NHS’s largest review exposes systemic failures

Senior midwife Donna Ockenden has released findings from the largest maternity review in NHS history, centred on Nottingham University Hospitals.

It says hundreds of mothers and babies died or suffered potentially avoidable harm. Experts concluded there were "potentially avoidable" outcomes relating to 444 maternity cases examined up to May 2025, alongside 76 newborn cases.

The report reveals systemic failures in leadership, governance, and culture, where bullying was tolerated, concerns were suppressed, and the voices of vulnerable women were dismissed.
