Children’s commissioner for England, Dame Rachel De Souza says that Sara Sharif was "totally failed" by the social care system, and that "services need to be better" at preventing these tragedies from happening again.
Ms De Souza says "I absolutely believe there has been failure in the services that should have supported her" and she calls for the government to take "real action" in bettering these systems.
